I prepared a checklist to get all of my OUPV
license application together. This checklist and associated files are
referenced, below. If you don't
have Word or something that can read it, let me know and I'll format it in
something else.
The files are
- my checklist
- CG application (CG719B)
and a sea service form (CG719S) attached - I used Word and set the margins and
tabs to type onto the form you can use word and just change the information.
the tabs are already set.
- oath form to take to
the local safety station to swear your oath
- character reference form
(used three long term friends)
- drug test locations
(call and they will tell you the local center)
- the drug test release
to fax back to DTC
- physical exam form (CG719K)

I used Mark Grossetti to expedite my application. I sent him the following
items, he reviewed them. There was a change he wanted made in my sea service
forms, which I made and sent him. Two weeks later I received my license in the
mail. Simple. I don't know if it really takes Mark to smooth the way at the
Coast Guard. I suspect if you just take the listed items to the Coast Guard you
wouldn't have much trouble. The extra money to Mark is to just eliminate a
little bit of the hassle. If you want to use him, his info is:
